The driver of a Colorado pickup truck did not respond to a flagger and entered the work zone.

Edward J. Ladouceur, 87, has been charged with motor vehicle homicide in the death of a construction worker in Pittsfield, Massachusetts in October. According to charges, Ladouceur struck and killed Shane Cassavant in a work zone.

According to a report in The Berkshire Eagle, Cassavant was going to retrieve a tool, a co-worker was directing traffic around the work site, and a third was operating an excavator. Ladouceur, driving a Chevrolet Colorado pickup truck, did not respond to the directions and drove into the work zone, striking Cassavant.

The news report also cited reports that traffic cones were present but no signage. OSHA is investigating.
