Contractor Fined $5K in Worker Death

A defective wheel loader caused the death of a worker and injured another in January, and the state fined Munitor Construction $5,400, according to news reports.

According to an article on OregonLive.com, the wheel loader was carrying gravel and did not have working brakes or horn. The operator attempted to avoid the two workers, but state investigators believe the loader hit a nearby excavator and caused the bucket to swing out, hitting both workers.

The loader then drove over one of the workers, killing him.
