OSHA cited a company for eight serious safety violations after a 28-year-old working relining a concrete dump hopper fell 17 feet through the hopper’s opening, suffering several fractures.

Inspectors found workers were exposed to permit-required confined space hazards because the company failed to:

  • Use proper equipment for entry and exit;
  • Use a retrieval system;
  • Have an attendant present during entry into confined spaces;
  • Post warning signs; and
  • Issue permits to enter such spaces.

Proposed penalties total $51,000.
