CBS Pilings Solutions, Riverside, MO, has been fined $14,700 for three safety violations after a worker was killed in a crane accident.

OSHA cited CBS after a 42-year-old worker was struck in the head by a piece of rebar that fell from a crane. The company was drilling foundation holes for a building under construction, and the worker was cleaning the area around the top of the hole when the incident occurred.

Citations were issues for improper rigging of the load, failing to train workers in struck-by hazards associated with working around cranes and suspended loads, and for not removing all nonessential employees for the fall zone during crane operations.
