It's called a Ladder Walk-Through Railing System, which attaches to the top of most extension ladders and is designed to enhance the ease and safety of exiting and entering the top of the ladder. It's made of aluminum and, according to the manufacturer, it exceeds OSHA's 36-inch extension rule for portable-ladders.
